Advertisement for Bid for Construction of Downtown Streetscape Project for City of Louisville, Georgia

Sealed bids will be received by the City of Louisville at the Louisville City Hall located at 1011 Peachtree Street, Louisville, Georgia 30434, until 2:00 on Friday, June 22, 2012 for the construction of Louisville Streetscape Project Phase 2 in the City of Louisville, Jefferson County, Georgia. GA. D.O.T. Federal Aid Project CSTEE 0009-00(098), P.I. Number 0009098. Bids received after the designated time will not be accepted. All bids must be originals; no bond copies, facsimile copies, or electronic copies will be accepted. No bidder may withdraw her/his bid within sixty-days (60) after the actual date of the opening thereof. The owner reserves the right to waive any informalities and any technicalities, and to reject any or all bids.

The project generally includes furnishing all labor, materials and equipment for the construction and related improvements for the Louisville Streetscape. The project is located along the median and sidewalk along the southwest side of Broad Street between Walnut Street and Green Street . This project will consist of, but is not limited to, erosion control, demolition of the existing pavement, maintenance of traffic, grading, excavation, subgrade preparation, and construction and installation of sidewalks, brick paving, ramps, curbing, walls, tree wells, plantings, signage and striping. Coordination with utilities will be required.

The successful bidder shall commence work with adequate force and equipment on a date to be specified in a written order of the City of Louisville, and shall complete the work within 180 consecutive calendar days from and including said date.

This is a re-bidding of a project that was originally bid on April 16th, 2012. Plan holders from the original bid are not required to purchase a new set of plans and specifications.

ONLY FULL & COMPLETE SETS OF BIDDING DOCUMENTS WILL BE ISSUED. NO PARTIAL SETS OF BID DOCUMENTS WILL BE ISSUED. COSTS ARE NON-REFUNDABLE. No exceptions will be made to this requirement.

No bid proposal will be considered unless accompanied by a certified check or acceptable Bid Bond in an amount not less than five percent (5%) of the bid and made payable to the City of Louisville. Successful Bidder shall be required to furnish a Contract Performance Bond equal to 100% of the contract price and a Payment Bond equal to 110% of the contract price, with the terms and surety to be approved by the City of Louisville; and furnish satisfactory proof of carriage of the insurance required.

The Disadvantaged Business Enterprise (DBE) goal for this project is 5%. All DBE firms must be certified with the Georgia D.O.T. Equal Employment Opportunity Office.

Bidders submitting bids over $2,000,000 shall be prequalified with the GDOT. Bidders submitting bids $2,000,000 or less shall be prequalified or registered subcontractors with the GDOT.

NOTICE OF THE OPPORTUNITY FOR PUBLIC COMMENT GEORGIA DEPARTMENT OF NATURAL RESOURCES ENVIRONMENTAL PROTECTION DIVISION AIR PROTECTION BRANCH STATE OF GEORGIA COUNTY OF JEFFERSON NOTICE OF DRAFT TITLE V OPERATING PERMIT To All Interested Parties: The Georgia Environmental Protection Division announces its intent to issue a Title V Air Quality Operating Permit to KaMin - Wrens Calcine Plant. The facility is located at 2069 Howard Mill Road, Wrens, Georgia. The primary purpose of this permit is to identify and consolidate existing State and Federal air requirements applicable to KaMin - Wrens Calcine Plant and to provide practical methods for determining compliance with these requirements. The Georgia Environmental Protection Division (EPD) is preparing Title V Operating Permits in accordance with Title V of the Clean Air Act. This permit will be enforceable by the Georgia EPD, the U.S. Environmental Protection Agency, and other persons as otherwise authorized by law. The operation of a kaolin processing plant. The draft permit and all information used to develop the draft permit are available for review. This includes the application and all other relevant materials available to the permitting authority. This information is available for review at the office of the Air Protection Branch, 4244 International Parkway, Atlanta Tradeport-Suite 120, Atlanta, Georgia 30354. Copies of the draft permit, narrative, and permit application will be available through our internet site http://www.georgiaair.org/airpermit/html/permits/draft. (Please note that the Internet is generally accessible from most public libraries in Georgia.) If copies are desired, a copying machine for public use is provided by EPD at the Atlanta Air Protection Branch Office. There will be a copying charge of $0.25 per page. Copies will be made on a first-come, first-served basis. Files are available for review and copying 8:30a.m. to 4:00p.m., Monday-Friday, excluding holidays. You are hereby notified of the opportunity to submit written public comments concerning the draft Title V Air Quality Operating Permit. Persons wishing to comment on the draft Title V Operating Permit are required to submit their comments, in writing, to EPD at the above Atlanta Air Protection Branch address. Comments must be received by the EPD no later than 30 days after the date on which this notice is published in the newspaper. (Should the comment period end on a weekend or holiday, comments will be accepted up until the next working day.) The Division, in making its final decision to issue the Title V permit, will consider all comments received on or prior to that date. Any requests for a public hearing must be made within the 30-day public comment period. A request for a hearing should be in writing and should specify, in as much detail as possible, the air quality related issues that constitute the basis for the request. Where possible, the individual making the request should also identify the portion(s) of the Georgia Rules for Air Quality Control and/or the Federal Rules that the individual making the request is concerned may not have been adequately incorporated into the draft permit. If a public hearing on the draft Title V Operating permit is to be held, the date, time and location of the hearing will be provided in a subsequent, separate public notice. At that hearing, anyone may present data, make a statement or comment, or offer a viewpoints or argument either orally or in writing concerning the draft Title V permit. During the Division’s decision-making procedures to issue the Title V permit, any information received during a public hearing will be considered together with information received during the comment period. Following this 30 day public comment period, the draft permit will undergo a 45 day review by U.S. EPA in accordance with 40 CFR 70.8 Information pertaining to U.S. EPA’s review period can be found on their website at www.epa.gov/region4/air/permits. After the comment period has expired, the EPD will consider all comments, make any necessary changes, and issue the Title V Operating Permit. Copies of the final Title V Operating Permit, comments received, EPD’s responses to comments, and any other relevant information will then de made available for public review during normal business hours at the office of the Air Protection Branch. A summary of comments and any changes made to the final permit will be found in an addendum to the narrative. A copy of this narrative and the final Title V permit will be added to the information already available at the internet address given above.
